Winter Sports Interscholastic Participation Agreement

Dear Parents:

Starting Tuesday October 9, 2007 we will begin signups for the winter sports season. The sports offered in the winter are: Wrestling, Boys and Girls Basketball, Boys and Girls Bowling, and Cheerleading. The sign up procedure is as follows. Permission slips will be handed out starting 10/9/07 in all Phys. Ed. classes. These forms must be brought home and signed by a parent or guardian and returned to the School Nurse’s Office by Wednesday 10/17/07. No permission slips will be accepted without penalty after 10/17/07. All students who wish to play must return a permission slip regardless of their need for a physical. Those students who do need a physical will have one scheduled by the nurse.

If you wish to have a physical performed by a private physician, make sure your son or daughter picks up the appropriate form from the School Nurse to take to your doctor. These completed forms must be returned to the Nurse’s Office prior to October 30th, 2007. To be clear, please understand that even if you are scheduling your own physical the permission slip explained in the prior paragraph still needs to be turned in by 10/17/07. I encourage you to have the school arrange the physical so that all physicals are completed on time.

Students who are 7th and 8th graders and wish to try out for a JV or Varsity sport have one additional requirement. These students must take and pass the NYS required Selection Classification Test. There is a separate permission slip which must be completed to allow us to test your son or daughter. This form is available from Ms. Dobony in the Middle School Phys. Ed. Office. This permission slip must also be completed and turned in by 10/17/07.
